Last year we announced the arrival of the PHQ-9 depression screening in our Hush™ Secure Forms template directory. This was the first of our calculated self-assessments that deliver a score upon completion. Today, we’re excited to announce the arrival of our next secure calculated form, the GAD-7.
What is the GAD-7?
The GAD-7 is a self-assessment tool that can be used to screen for, diagnose, and assess the severity of generalized anxiety disorder. It’s made up of seven stated problems such as - feeling anxious, nervous, or on edge. The person taking the assessment chooses one of four responses to each problem according to how often that condition is experienced - not at all, several days, more than half the days, nearly every day.
A final question assesses the effect of these problems on quality of life:
If you checked off any problems, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people?
The assessment is scored and recommendations are provided.

The GAD-7 joins the PHQ-9 depression screening form in our template directory, and we’re excited about adding more calculated forms very soon. Next up will be the PHQ-9A, a version of the depression screening for children. Also in the works are the GAD-7 for adolescents, the Columbia Suicide Severity Rating Scale (C-SSRS), and the Screen for Child Anxiety Related Disorders (SCARED).
