Templates and examples to kickstart your web forms

If you’re building a web form for the first time, you might find staring at a blank page a little daunting. Building a web form is easier if you start from a tested example or template.

However, you might find staring at a blank form a little daunting. Where do you start? Fear of the blank page can get to anyone: writers, artists, and web form builders alike. Fortunately, when you’re a practitioner/web form builder, there’s an easy solution. Don’t start from nothing. Start from a tested example or template.

Starting from an example or template minimizes your effort

Starting from something means you don’t have to spend time writing and organizing all the content yourself. There’s no reason to reinvent the wheel, or web form, when that time would be better spent caring for your clients. Templates and examples allow you to see what a form might look like upfront so you can more easily plan out the content. Examples and templates can also help ensure you don’t accidentally omit important sections, giving you confidence when you have to build and publish a form quickly.

How to customize examples

If you choose to use one of the above examples, you’ll want to pay attention to a few important modifications so the form accurately reflects your practice. 

  • Look for sections in brackets such as [PRACTICE NAME] and replace with content specific to your practice
  • Consider adding information that would benefit your clients. For example, if you use the form modeled on Carol Park’s Technology consent to treatment form, you might consider adding a section giving details about the teletherapy platform you use. 
  • Add a signature field if one is needed
  • Add your logo or headshot, bio, and contact information
