Customize our 3 most popular web form templates

We are often updating our directory with new templates. In today’s post, we’ll tell you about the three templates our customers use the most and make recommendations about how to customize them.


Template directory

Hush™ Secure Forms comes with a top-notch, drag-and-drop form builder, but for those of you who don’t want to build a form from scratch, we provide a growing template directory full of popular forms you can use immediately. 

You can use a template just as it is, or you can use it as a starting point to build your own form. You can even add conditional visibility to the fields in our templates, creating a completely customized version to meet your practice’s unique needs.

We are often updating our directory with new templates. In today’s post, we’ll tell you about the three templates our customers use the most and make recommendations about how to customize them.

Secure Contact Form

The template that our customers use the most is our Secure Contact Form, which can be found in the general section of our directory. Your clients can use this form to initiate a secure conversation with you.

This is the form we recommend when you need to redirect prospective clients away from a contact form that isn’t secure, such as the one that comes with your Psychology Today profile. If you disable the profile contact form, which isn’t HIPAA compliant, and link to your Secure Contact Form instead, you can then assure prospective clients that their information is safe the minute they reach out to you.

Our contact form is very basic, but you can customize it with additional fields so you can collect the information you need. 

For example, you might decide you want to screen your potential clients for depression and anxiety. You can do this by setting up conditional visibility so certain keywords, such as “anxiety” or “depression,” cause a screening link to one of our calculated forms like the PHQ-9 to show up. 

We also provide digital versions of the GAD-7 anxiety screening and DASS depression, anxiety, stress scales in our directory. 

If you use conditional visibility, the calculated form link will only appear on your published form if your prospective client mentions the keywords in their explanation about how they’d like you to help them.

New Client Questionnaire

Our New Client Questionnaire (Adult) is an intake form for adult clients. You can send this questionnaire to your new clients, to be completed prior to their first appointment. 

Just like with the contact form, you can add more fields to collect the information you need. You can also turn this form into a questionnaire that performs double duty, tailoring itself to an adult or child depending on how your client fills in the date of birth field.

In the example below, we’ve added parent/guardian contact fields that only show up if the client is younger than 18 years old. If you make these changes to our template, be sure to remove “adult” from the title.

COVID-19 Screening Questionnaire

The COVID-19 Screening Questionnaire may be given to your clients to complete before their appointment. 

Because the recommendations for COVID-19 screening may change depending on where you care for your clients, it may be necessary to adjust the questions to comply with local health advice. For example, you might want to add the symptoms that require immediate medical attention along with a prompt to do so if those symptoms are checked.
